JOB OPPORTUNITY FOR YOUNG MEDICAL DOCTORS

More than 50% of all medical students enrolled in Tanzania between 2011 and 2020 will not be practicing medicine by 2025 and they won't be captured anywhere as medical doctors in public health facilities. This is due to failure of government to absorb them, past two years at least 1,774 young doctors have not recruited. The cost of training one doctor is estimated at $25,000 (Sh 55 Million), Due to failure of government to absorb them; the loss was estimated at Sh 68.3 billion until last year. Tanzanias doctor-to-patient ratio is pegged 1:25,000 (The Citizen March 20, 2017). IDEA ON THE SOLUTION The core of this idea is based on widening the act of self-employment in health sectors where it seems to be underexploited by the community especially health practitioners themselves. Three to five group of licensed doctors may organize and open new health centers/dispensaries/diagnostic centers with cooperation from private sectors/Health Insurance Funds for keeping dreams of young doctors, provide employment, improvement of health sector. Major stakeholder to this is Health insurance funds, as they accept working with these new open health centers/dispensaries, accepting their customers to receive services from these new centers opened by young doctors and refund them through insurance system. Also private sector/investors could use the opportunity by collaborating with these jobless doctors provide the funds and work under their companies/contracts that will benefit to both. For example in Tanzania it cost $ 6,000 to 8,000 to open a dispensary in area where it can serve up to 10,000 people, and salary for a medical doctor is $ 560 per month, if these 10,000 people will be attended at the dispensary and pay $ 4 per month through insurance funds, a total of $ 40,000 will be collected per month and able to pay 70 Doctors.
Expected impact of your idea on sustainable development
After the UN's Millennium Development goals have reached their end, progress has been made toward improving better life to the community in the world through 17 Sustainable Development Goals (SDGs). The idea of Job Opportunity to Young Medical Doctors will directly bring changes to most of our communities and be the solution to challenges the community face in provision of quality health services to all groups. Though new health centers/dispensaries/diagnostic centers that will be operated by the small groups made by the new licensed doctors, it will; 1. Create job opportunity to them (Doctors) and other other health practitioners (Nurses, Lab Technicians) that government failed to absorb them despite the large amount of money invested to them during their training. 2. Increase number of health centers as well as active health practitioners where people can easily access health services hence improve health of communities and able to attain SDGs. 3. Keep and maintaining dreams/ambitions of the young doctors in medical arena as they will actively continue to practice medicine and hence motivate them for acquiring further knowledge and finally have many medical specialist in future required to provide quality health services. 4. Create opportunity to private sectors on investing on health system in terms of enhancing the growth of health sector trough providing loan/grants to these young doctors, build health centers, job opportunities. 5. Help in overall community Economic growth through increase tax collection from these new health centers, hence increase national income for improving other sectors(Education, infrastructures)
Plans for implementation and sustainability
The plans for the idea include: 1. Fund collection. Doctors themselves who wish to put the ides into action could contribute to have the fund or through Private/investors partnership an especially Health insurance funds. These insurance could provide loans to these group and have a contract to refund it in terms of attending their customers in these new health centers/dispensaries. 2. Survey. This will help to identify health situational analysis and need for health centers to the specific areas for proper distribution of health services as well as sustainability of new centers by ensuring the availability of people who will need the services. 3. Equipments installation. It includes buying of the all health equipments required in the center, laboratory equipment, furniture, pharmacy (medications), and stationaries 4. Registration. They should be registered to the corresponding authorities/boars so as to be recognized, include quality insurance of the services provided, paying taxes and overall regulation of qualifications for operation of these centers 5. Opening. these young doctors can attend the patient through these new open health centers/dispensaries/diagnostic centers. Proper management of patients will be enhanced by availability of the qualified health practitioners, reliable diagnostic equipments, definitive management (medical/surgical) and supportive care. 6. Assessment. This is done by looking on the attained goals/plans as well as assessment of overall work done by the staffs and raising the challenges that might be threating the growth and sustainability of these centers. SOURCE OF INCOME 1. Refund from the Health insurance fund that accepted to work with the new health centers/dispensary/diagnostic centers after attending their customers. 2. Cash payment by patient at the centers after receiving services.
